The pull down to the back of the neck is a commonly used exercise in many gyms, and is a variation of the pull down exercise. Bringing the bar to the rear of the head results in the upper arms being more perpendicular to the torso, therefore leading to shoulder adduction which stimulates the latissimus dorsi muscle greatly. The behind the neck pull down does place the shoulder joint in a possibly troublesome position however, and those with injurious rotator cuffs may wish to avoid this exercise.

Brining the bar to the back of the head avoids the arms being brought towards the front of the torso which would result in the extension of the shoulder. The upper arms being flared outwards at ninety degrees to the torso results in the adduction of the shoulder. The major muscles of the back will be recruited during the behind the neck pull down, including the latissimus dorsi, rhomboids, and the lower and middle trapezius.

Many trainers avoid the behind neck pull down due to the unnatural position of the body during the lift which may be hazardous. Those with injurious necks, backs, elbows or shoulder joints may wish to totally avoid this exercise. Those who opt to perform this exercise should pay close attention to their form during the execution of the lift. Tilting the head excessively forward can cause injury, and so can excessive jerking. A choice of many is to bring the bar to no further than ear level, avoiding the main hazardous zone where the bar is brought down towards the back of the neck.

Those new to the exercise would be recommended to begin with a light weight, ensuring the form of the exercise is mastered before progressing onto a greater weight. More advanced trainers can progress using greater resistance on the pull down station, or alternatively perform the exercise on a pull up station with extra resistance added to a chinning belt.

Gains in muscle growth are obtained with a repetition range typically between eight and twelve. Due to the nature of the exercise, it would be wise to avoid excessive weight with low repetitions when performing this exercise. Those who seek to build muscle strength may wish to perform the chin up or pull up using greater resistance and low repetitions, typically between one and six per set.

There are a myriad of ways to treat a frozen shoulder these days, some of which are good and some which may leave the sufferer with more pain and loss of motion than they started with. This article will cover a couple of key components to successful frozen shoulder treatment, and a few treatment programs to avoid.

The first key element in any treatment program is consistency. Whether you are in your mid twenties (yes, this age group suffers as well), a baby boomer, or a senior in the 60 plus range, you must stay consistent with the prescribed treatment on a daily basis for at least 3 weeks before abandoning the program for another one, or worse, quitting altogether. Now if you're seeing increases in range of motion or function after 3 weeks, you may need to stay with your program another 4-5 weeks for maximum gains. The reason is that it takes up to 3 weeks to recover from initial irritation from stretching or breaking loose adhesions that may have formed in the joint capsule.

The second key element in successful frozen shoulder treatment is intensity. All too often I see patients "going through the motions" of a well designed program without sufficient intensity and become discouraged because lack of results. Correct intensity varies from one individual to another but can easily be learned in the first few sessions. Thinking of the phrase "medium-to-strong, but not fully strong" and apply it to each exercise. For instance, if you are stretching in a particular direction, you should feel a "medium-to-strong" stretch, but not fully strong or unbearably painful, and hold that stretch.

As far as unsuccessful treatments, a few guidelines apply as to which ones to avoid:


Programs that sound gimmicky (i.e holding the non-burning end of a lit candle in your ear will probably not help your frozen shoulder) Programs that are not research-based or evidence-based (i.e. programs where the author has no credentials) Programs that promise instant relief of symptoms (i.e. "Cure your frozen shoulder overnight!") Programs based only on relief of symptoms and no focus on return of function

These are just a few red flags to watch for, however there are a few good programs available and would encourage anyone seeking frozen shoulder treatment to make sure they meet some of the criteria above.

Following are three tips that you can use when receiving physical therapy not only for a knee replacement but after any orthopedic surgery for that matter.

1. Turn off all media outlets. There is nothing more disturbing to a physical therapy session then to have a radio or television that you have to compete with. Though you may be watching something that is important to you, your full concentration cannot be given with one eye or ear to the media outlet and another on to your rehabilitation professional. You will be expected to learn the exercises to complete on your off days from rehab so, getting the lesson down with a full understanding is vital if you want to succeed and see results fast.

2. Keep the animals out of the room. Yes this is a big one. Everyone loves their animals however you will be hard pressed to concentrate on instruction and feel the appropriate muscles working with the dog or cat jumping into the middle of the exercise session. This happens more then you can imagine. To fully concentrate and actually visualize the knee and other muscular structures working, you need to keep your eye on the body part and not on the pet that may jump on that fresh incision.

3. Keep family members out. Now this is a tricky one. keeping out family members include the kids for instance that cannot settle down or the brother or sister that cannot help but share their personal problems or aches and pains with the physical therapist. After all they are there to help you. Now there is absolutely nothing wrong with a caregiver, spouse, or friend that is assisting you with your exercises and would like to observe to sit and learn. it is welcomed for the most part and there are times when having someone there can be reassuring just for moral support.

Keeping your full focus and total concentration during your exercise session is simply a matter of reducing outside interferences so that your session is efficient and productive. Keeping focused then allows you to connect your mind to the muscle groups being exercised which in turns magnifies your results.
